Add the title "Testament" or "Last will"
Add date and signature
Deposit the will in a safe place and inform a trusted person

Living will:

Deposit the document in a safe place and inform a trusted person

Health care proxy

Click on the yellow button and fill in the advance care directive online
Print out the document and copy it by hand
